SUPERVISOR’S TITLE: Healthy Families Supervisor
POSITIONS SUPERVISED: None
JOB STATUS: Full Time, Non-Exempt
SCHEDULE: Monday- Friday, 8 am to 4 pm; Must be flexible to complete evening visits as needed by families.
Summary

This role builds strong, trusting relationships with families while connecting them to resources and supporting long-term family stability.
Key Responsibilities
- Conduct home visits, assessments, and ongoing contact with families in accordance with program standards
- Establish and maintain trusting relationships with families through consistent engagement
- Provide education on child development, parenting skills, and family well-being
- Assist families in developing and implementing Individualized Family Support Plans
- Conduct developmental and family assessments, including screenings as required
- Connect families with community resources and follow up on referrals
- Support parents in scheduling and attending appointments and accessing services
- Maintain accurate and timely documentation, including case notes and database entry
- Manage assigned caseload and maintain organized client records
- Provide crisis intervention and report concerns to supervisor immediately
- Maintain regular communication with referrals and assist with program enrollment
- Assist with parent group activities and community events
- Travel within assigned service areas to meet family needs
- Promote child safety and report any concerns related to abuse, neglect, or violence
- Strong interpersonal skills with the ability to build trust and rapport with families
- Understanding of child development, family dynamics, and parenting challenges
- Ability to work effectively with culturally diverse populations
- Strong communication skills, both written and verbal
- Crisis intervention and problem-solving skills
- Strong organizational and time management skills
- Ability to work independently and manage a flexible schedule
- Basic computer skills for documentation, data entry, and reporting
- Reflective practice skills and openness to supervision and professional growth
- Ability to travel frequently for home visits (up to 90 minutes one way; occasional longer travel for training)
- Ability to sit on the floor during home visits and sit for extended periods for documentation
- Ability to lift and carry supplies as needed
- Valid driver’s license, reliable transportation, and proof of insurance required
- Completion of Healthy Families Indiana Core Training and FROG training
- Completion of required on-site program trainings within designated timeframes
- Infant Mental Health endorsement preferred
- CPR and First Aid certification
- Preventing Disease Transmission training
