PURPOSE OF POSITION:
Peer Support Services are structured and scheduled activities for adults aged eighteen (18) and older with a diagnosis of Mental Health and Substance Use disorders. Peer Supports are provided by Peer Support staff. Peer Support Service is an individualized, recovery-focused service that enables individuals to learn to manage their own recovery and advocacy process. Interventions of Peer Support staff serve to enhance the development of natural resources, as well as coping and self-management skills. Interventions of Peer Support staff may also provide supportive services to assist an individual in community re-entry following hospitalization.
Peer Support Services emphasize personal safety, self-worth, confidence, and growth; connection to the community; boundary setting; planning; self-advocacy; personal fulfillment; and the development of social support and effective communication skills. Services emphasize the acquisition, development, and expansion of rehabilitative skills needed to move forward in recovery.
Specific interventions include:
· Self Help: Cultivating the individual’s ability to make informed, independent choices. Helping the individual develop a network of contacts for information and support based on experience of the Peer Support staff.
· System Advocacy: Assisting the individual to talk about what it means to have a mental illness to an audience or group. Assisting the individual with writing a letter or making a telephone call about an issue related to mental illness or recovery.
· Individual Advocacy: Discussing concerns about medication with the Physician or Nurse at the individual’s request. Helping the individual make appointments for psychiatric and general medical treatment when requested. Guiding the individual toward a proactive role in health care.
· Pre-Crisis and Post-Crisis Support: Assisting the individual with the development of a personal crisis plan, and/or a Psychiatric Advance Directive (PAD). This includes help in developing the Wellness Recovery Action Plan (WRAP). Giving the individual feedback on early signs of relapse and how to request help to prevent a crisis. Assisting the individual in learning how to use the crisis plan.
WORK SCHEDULE:
Work schedule will vary per assigned Certified Peer support staff.
Our ideal Qualified Professional is an excellent communicator and cooperative team player who excels at interviewing the people we serve to learn about their interests and goals, even individuals who cannot communicate verbally. You must also be detail-oriented and have excellent written communication skills to develop and disseminate person-centered plans.
The QP will provide service activities to include the following interventions:
· Monitor service provision of PSS and CPSS.
· Provide education and training to CPSS related to crisis response, symptoms, behaviors, goals, intervention in the Person-Centered Plan.
· Assisting CPSS by finding resources, advising the consumer of processes, and encouraging follow-through with proposed resolutions, locating social activities, or providing other assistance as needed
· Provide preventive and therapeutic interventions designed for direct individual activities.
· Assist with skill enhancement or acquisition, and support ongoing treatment and functional gains
· Therapeutic mentoring and Supportive counseling.
· Symptom monitoring and self-management of symptoms
· Inform the beneficiary about benefits, community resources and services
· Assist the beneficiary in accessing benefits and services
· Arrange for the beneficiary to receive benefits and services
· Assist with case management to arrange, link, or integrate multiple services as well as assessment and the reassessment of the beneficiary’s need for services, are conducted within 6 months to annually
· Provide coordination of movement across levels of care, directly to the person/family, Referral linkage, and Person-Centered Planning
· Coordinates discharge planning and completes discharge paperwork.
· Coordinates community re-entry following hospitalization, residential services, and other levels of care.
· Service coordination activities with other providers such as school, psychiatric services, therapy services, vocational, medical, department of corrections to name a few.
· The QP will be responsible for required documentation such as documentation within 24 hours (or as determined by the agency) ongoing updates of PCP/crisis plan, facilitating team meetings, and conducting monthly supervision or as needed.
· Help with developing and updating the Person-Centered Plan (PCP) with the individuals being served, and natural support.
· Provides input into PCP modifications, as needed.
· Ensure authorization remains current and meets appropriate medical necessity
· The QP is responsible for making sure that the required documentation is turned into medical records by the timeframe specified in the policy.
· Ensure that the CPSS is completing the WRAP, Recovery, and screening (RAS) tool every 90-day basis.
· Complying with the team approach, which includes the beneficiary, CPSS, therapist, and QP.
· Participate in scheduled Utilization Reviews with Program manager, Clinical Director, and QA/QI Director.
· Provide quality review and approval of CPSS service notes as appropriate, checking them against goals as delineated in PCPs.
· Other tasks assigned or requested by supervisor.
Additional requirements for Qualified Professional (Peer Support) include:
· Bachelor’s degree in social work, social services, human services, or Special Education required.
· Master’s degree in social work, social services, human services, or Special Education required.
· Minimum 2 years’ experience working with people with population served is required; 1-year experience if paired with a master’s degree
· Working knowledge of applicable regulations that apply to our service offerings
· Valid driver’s license, auto insurance, and reliable transportation
· Ability to pass a drug screen and background check
· Ability to lift between 20 and 50 pounds and meet the physical requirements of performing CPR
· Supervisory/management experience and experience writing person-centered (individualized service) plans preferred
· Must complete a minimum of twenty (20) hours of training specific to the required components of the specific service definition including crisis response, within the first 90 days of employment
· Valid driver’s license and automotive insurance
· Ability to pass background checks and drug screen
· CPR, first aid, NCI and/or role-related certification, a plus
Pay: $23.07 - $27.00 per hour
Expected hours: 20.0 – 30.0 per week
