- Perform in-home assessments and speech-language evaluations
- Develop and implement individualized treatment plans
- Provide therapy for communication, cognition, voice, and swallowing disorders
- Educate patients and caregivers on exercises and communication strategies
- Maintain accurate documentation in compliance with home health regulations
- Collaborate with interdisciplinary team members such as nurses and physicians
- Master's degree in Speech-Language Pathology from an accredited program
- Active California SLP license and current CPR certification
- Minimum of 1 year experience, preferably in home health or medical setting
- Valid driver's license, reliable transportation, and willingness to travel within service region
- Excellent communication, interpersonal, and time management skills
- Ability to work independently and collaboratively
- Ability to sit, stand, bend, lift (up to 50 pounds), walk, and move intermittently
- Must be able to drive regularly
- Passing a medical exam is required before final acceptance
