Speech Therapist

Role: Motivated Speech Therapist providing in-home care and case management for clients across multiple counties in Northern California.

Responsibilities

• Perform in-home assessments and speech-language evaluations
• Develop and implement individualized treatment plans
• Provide therapy for communication, cognition, voice, and swallowing disorders
• Educate patients and caregivers on exercises and communication strategies
• Maintain accurate documentation in compliance with home health regulations
• Collaborate with interdisciplinary team members such as nurses and physicians
  
  

Qualifications & Skills

• Master's degree in Speech-Language Pathology from an accredited program
• Active California SLP license and current CPR certification
• Minimum of 1 year experience, preferably in home health or medical setting
• Valid driver's license, reliable transportation, and willingness to travel within service region
• Excellent communication, interpersonal, and time management skills
• Ability to work independently and collaboratively
  
  

Physical & Other Requirements

• Ability to sit, stand, bend, lift (up to 50 pounds), walk, and move intermittently
• Must be able to drive regularly
• Passing a medical exam is required before final acceptance
