Key Responsibilities
- Conduct evaluations and develop individualized care plans
- Deliver therapy sessions to support wellness, longevity, and functional independence
- Document patient progress accurately in electronic medical records
- Collaborate with a multidisciplinary team including PTs, SLPs, and fitness specialists
- Educate patients, caregivers, and staff on safety and aging-in-place strategies
- Utilize telehealth and virtual care to extend access and flexibility
- Valid Occupational Therapy license and degree from an accredited program
- Proficiency with computers and EMR systems
- Current CPR certification
