BHcare is seeking a compassionate, proactive Housing Case Manager to support individuals with a history of opioid use who are experiencing homelessness or are at risk of becoming homeless. This vital role helps clients navigate the housing system, connect with critical resources, and build long-term stability and recovery.
This is a full-time position, 37.5 hours a week, Monday-Friday 8:30-4:30 PM the time can have flexibility.

Through multidisciplinary collaboration, evidence-based practices, and harm reduction principles, we empower individuals to achieve stability and wellness.
Position Summary
The Housing Case Manager works directly with individuals who have opioid use histories to help them obtain and maintain stable housing. This role blends outreach, advocacy, and intensive case management to reduce barriers to housing and promote long-term stability. The Housing Case Manager collaborates closely with internal clinical teams and community partners to support recovery and prevent homelessness.
Key Responsibilities
Client Engagement & Support
- Build trusting, supportive relationships with clients through consistent contact
- Assist individuals in identifying housing options and navigating the rental process
- Support clients in developing individualized housing stability and recovery plans
- Connect clients to community-based services and assist with housing transitions
- Respond flexibly to evolving client needs, including crisis situations
Collaboration & Coordination
- Partner with internal programs, housing providers, and community agencies
- Facilitate access to housing, benefits, employment, and healthcare resources
- Participate in regular case reviews, team meetings, and cross-program collaboration
Documentation & Reporting
- Maintain timely, accurate documentation in the electronic health record (EHR)
- Track progress toward client goals and support program outcome reporting
- Ensure compliance with all documentation, confidentiality, and data standards
Program & Community Involvement
- Represent BHcare at community meetings, housing-related events, and with partner agencies
- Assist with transportation for clients when needed
- Deliver services using Housing First and harm reduction principle

Qualifications:
Education
- High School Diploma or equivalent required
- Bachelor’s degree in Human Services, Social Work, Psychology, or a related field preferred
Experience & Skills
- Experience working with individuals with mental health and/or substance use disorders
- Experience working with individuals who are homeless or from marginalized populations
- Demonstrated ability to engage effectively with diverse communities
- Working knowledge of Housing First and harm-reduction-based service delivery
- Familiarity with employment, housing, and human services systems in Greater New Haven strongly preferred
Technical Skills
- Proficiency in Microsoft 365
- Ability to navigate and document effectively within an Electronic Health Record (EHR) system
Training (preferred but not required)

Language Skills
- Bilingual Spanish preferred
Transportation Requirements
- Valid driver’s license required
- Must maintain an acceptable driving record
- Must be insurable at a reasonable rate under the organization’s auto insurance plan
