BHcare is seeking a compassionate and skilled Young Adult Services (YAS) Clinician to provide high-quality, evidence-based clinical care to young adults in both residential and outpatient settings.
This role is ideal for a clinician who is passionate about supporting individuals navigating mental health and substance use challenges during a critical stage of independence and recovery.
Working collaboratively with a multidisciplinary treatment team, the YAS Clinician plays a vital role in helping clients build resilience, achieve recovery goals, and transition successfully into adulthood.
This position is a full-time position, 37.5 hours per week. The schedule will be as follow: Monday, Wednesday and Friday 8:30am-5pm and Tuesday and Thursday 11:30am-8pm
Key Responsibilities
Clinical Care & Treatment
- Serve as the primary clinician for an assigned caseload
- Provide individual, group, and family therapy using evidence-based practices
- Conduct psychosocial assessments, intake evaluations, and develop individualized treatment plans
- Deliver crisis intervention and emergency assessments as needed
Care Coordination & Collaboration
- Collaborate with psychiatrists, treatment teams, families, and community providers
- Act as a liaison between clients and external systems (hospitals, benefits providers, community resources)
- Support discharge planning and continuity of care
Recovery Planning
- Develop and manage integrated, person-centered recovery plans
- Monitor client progress and adjust treatment plans accordingly
- Facilitate regular care reviews with clients and care teams
Documentation & Compliance
- Complete timely and accurate clinical documentation, including progress notes, treatment plans, and discharge summaries
- Maintain compliance with agency standards, payer requirements, and state regulations
- Manage insurance authorizations and utilization reviews
Program & Team Participation
- Participate in team meetings, case reviews, and program development
- Provide coverage and support across YAS residential and outpatient services
- Contribute to a collaborative and supportive team environment
What You Bring
Qualifications
- Master’s or Doctoral degree in Social Work or a related field required
- Connecticut licensure required (LCSW preferred; LMSW, LPC, or license-eligible candidates considered)
- Clinical experience in mental health and/or substance use treatment preferred
