The Case Manager demonstrates BHcare's commitment to providing compassionate, person-centered, trauma-informed services that promote dignity, recovery, housing stability, and community integration for individuals and families served.

This is a full-time position, 8:30 am to 4:30 pm Monday-Friday 37.5 hours per week. 

Housing Assistance and Supportive Housing Services

  • Assist individuals and families in applying for and maintaining housing subsidies and supportive housing programs funded through HUD, DMHAS, DOH, and other funding sources.
  • Guide applicants through the eligibility and application process, including obtaining required documentation, identification, verifications, and releases of information.
  • Assess and determine preliminary program eligibility in accordance with HUD, DOH, Coordinated Access Network (CAN), and agency guidelines.
  • Support clients throughout all phases of housing search and placement, including developing individualized housing search strategies.
  • Serve as an advocate for clients by working collaboratively with landlords, property managers, realtors, community partners, and service providers.
  • Negotiate housing opportunities and facilitate successful housing placements.
  • Conduct regular home visits, office visits, and telephone contacts to monitor housing stability and provide ongoing support.
  • Assist program participants in transitioning from homelessness to permanent housing and maintaining compliance with lease agreements, program requirements, and individualized service plans.
  • Participate in internal and community meetings related to homelessness prevention, housing programs, and coordinated service delivery.
  • Collaborate with community agencies to provide educational presentations, trainings, and informational sessions regarding housing resources.

Program Compliance and Housing Monitoring

  • Complete housing inspections and required program assessments, including:
    • Housing Quality Standards (HQS) inspections
    • Acuity Index assessments every six months
    • Annual reassessments
    • Rent reasonableness evaluations
    • Rent recertifications
    • Household eligibility reviews
    • Required participant notifications and releases
  • Maintain compliance with all federal, state, agency, and funding source regulations.

Direct Care and Case Management

  • Develop, implement, and review person-centered treatment and service plans with measurable goals and objectives at least every six months.
  • Monitor, document, and evaluate client progress toward established goals.
  • Develop individualized safety plans when clinically indicated.
  • Provide outreach, crisis intervention, and stabilization services as needed.
  • Facilitate individual and group educational sessions as assigned.
  • Promote health and wellness through education and linkages to healthcare and community resources, including HSMR when appropriate.
  • Provide support, education, and consultation to family members, natural supports, and community stakeholders.
  • Encourage and motivate clients to pursue goals related to employment, education, community integration, and recovery.
  • Assist participants in building self-advocacy skills and accessing community-based resources, peer support, and natural supports.
  • Maintain a minimum of 60% face-to-face direct service contact with clients.
  • Consult with the Program Manager regarding changes in client needs and appropriate levels of care.
  • Maintain flexibility to work evenings, weekends, and holidays as needed to meet client and program needs.

Quality Management and Documentation

  • Adhere to agency standards of care and quality improvement initiatives.
  • Ensure accurate, timely, and comprehensive documentation of all client services and program activities.
  • Maintain client records in compliance with agency policies, funding requirements, accreditation standards, and regulatory guidelines.
  • Monitor expiration dates of releases, consents, and required documentation and ensure records remain current.
  • Communicate and coordinate care with community providers and referral sources as authorized by the client.
  • Provide required data and reports to program leadership in a timely manner.
  • Attend agency trainings, workshops, conferences, supervision sessions, and professional development activities as required.
  • Maintain compliance with all applicable regulatory and accrediting bodies, including DMHAS and CARF standards.
Qualifications:

  • Bachelor's degree in Human Services, Social Work, Psychology, Counseling, or a related field preferred.
  • Experience working with individuals experiencing homelessness, behavioral health challenges, substance use disorders, or housing instability preferred.
  • Knowledge of supportive housing programs, housing subsidies, community resources, and case management practices preferred.
  • Strong organizational, documentation, communication, advocacy, and crisis intervention skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Proficiency with electronic health records and data management systems.
  • Valid driver's license, reliable transportation, and proof of automobile insurance required.
  • Ability and willingness to use a personal vehicle for agency business and client transportation when necessary.
  • Regular and dependable attendance required.
  • Ability to travel throughout the service area for home visits, inspections, client meetings, and community-based services.
