Social Worker Responsibilities & Duties
- Oversee the referral process for prospective residents
- Complete admissions process for all new residents
- Conduct psychosocial assessments for new residents
- Develop individualized care plans in collaboration with healthcare teams
- Provide individual and group counseling to residents
- Assist residents in adjusting to nursing home life
- Advocate for residents' rights and needs
- Facilitate communication between residents, families, and staff
- Coordinate discharge planning and follow-up services
- Assist families with understanding and navigating the healthcare system
- Organize and implement recreational and therapeutic activities
- Maintain accurate documentation and case records
- Participate in interdisciplinary team meetings
- Offer crisis intervention services as needed
- Provide support and resources for end-of-life care
- Ensure compliance with federal and state regulations
- Conduct family meetings and provide mediation when necessary
- Coordinate with community resources for additional resident support
- Educate staff on psychosocial issues affecting residents
- Monitor and evaluate the effectiveness of care plans

Nursing Home Social Worker Qualifications & Skills
- Experience in geriatric or long-term care settings
- Knowledge of Medicare and Medicaid regulations
- Strong communication and interpersonal skills
- Experience with case management and care coordination
- Ability to work independently and as part of a multidisciplinary team
- Familiarity with community resources and services for the elderly
- Proficiency in electronic health record (EHR) systems
- Skills in crisis intervention and conflict resolution
- Bachelor's degree in Social Work (BSW) or related field preferred
- Minimum of 1-2 years of experience in a healthcare setting
