This unique nursing positions offers flexibility around scheduling to meet both the staff and family’s needs. Use your excellent people, organizational, and communication skills to hit the ground running for this person-centered nursing position.

- Conduct routine home visits based around family’s needs.
- Complete all required documentation on individuals assigned, including health status reports, clinical assessments, and routine case notes.
- Conduct routine training for staff and families.
- Any other duties as assigned by the Director of Family Support and Advocacy
