Job Responsibilities:

• Assess and diagnose speech, language, and swallowing disorders in patients.
• Create and implement individualized treatment plans based on patient needs.
• Provide therapy services to patients in their homes.
• Educate patients and their families on speech therapy techniques and exercises.
• Collaborate with other healthcare professionals to coordinate patient care.
• Maintain accurate and up-to-date patient records.

Qualifications:

• Valid state license or certification as a Speech Therapist.
• High school degree or equivalent education.
• Prior experience in home health or a related field is preferred.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassionate and patient-centered approach to care.
