About the Role:
- Conduct comprehensive assessments of patients' physical abilities and develop personalized treatment plans
- Provide in-home physical therapy services to patients, including therapeutic exercises, manual therapy, and other interventions
- Collaborate with other healthcare professionals to ensure the best possible outcomes for patients
- Educate patients and their families on proper techniques for managing their conditions and achieving optimal health

Requirements:
- High school degree or equivalent
- Valid state license to practice as a Physical Therapist
- Prior experience in home health or a related setting is preferred
