Duties

• Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating patient needs.
• Administer medications and other treatments following established protocols and physician orders.
• Document patient information accurately using Electronic Health Record (EHR) systems like Epic or Cerner; ensure compliance with HIPAA regulations.
• Support specialized care for patients with complex conditions including Alzheimer’s disease, dementia, infectious diseases, and post-acute recovery.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans; participate in discharge planning and case management.
• Provide patient education on health coaching topics such as medication management, nutrition, pain control, and chronic disease management.
• Respond promptly to triage calls or emergency situations; assist with airway management and acute pain control when needed.

Qualifications

• Valid state licensure as a Licensed Practical Nurse (LPN) with current certification.
• Proven experience working in diverse healthcare environments such as hospitals (including ICU or Level I Trauma Centers), nursing homes, hospice care, or outpatient clinics.
• Familiarity with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks; strong medical documentation skills.
• Knowledge of ICD-9/10 coding for accurate medical record keeping and billing support.
• Experience with specialized care areas including geriatrics, memory/cognitive disorders (Alzheimer’s/dementia), pediatric or neonatal care preferred.
• Competence in IV insertion/insertion techniques and phlebotomy; ability to perform venipuncture safely and effectively.
• Strong understanding of physiology, anatomy, pharmacology, infection control protocols and HIPAA compliance standards.
• Ability to work independently and as part of a multidisciplinary team in fast-paced environments such as emergency rooms or surgical units.
