Beloit Health System at Beloit Regional Hospice is hiring for a Social Worker- Hospice/Palliative Care to join our team.

In this position the Social Worker will play a crucial role in supporting patients with serious, life-limiting illnesses and their families. The responsibilities of this Social Worker will focus on emotional, social, and practical aspects of care to improve quality of life including: emotional and psychosocial support, care coordination and advocacy, advance care planning, family and caregiver support, resource connection, and financial assistance.

Requires:

• Completion of a Masters Degree in Social Work

• Current WI or IL Licensure, with ability to obtain the other (must apply within 30 days of hire)

• Hospice or Palliative Care experience is preferred
