Pay: $22 - $24 per hour

The Case Manager is responsible for coordinating and monitoring service delivery for individuals served in Support Services by completing case management functions, facilitating person-centered planning, and ensuring required documentation and communication with internal and external stakeholders
Responsibilities Include
- Initiating, supporting, and promoting quality of life for clients through empowerment, safety awareness, respect, and dignity.
- Coordinating and participating in Health Care Management meetings, accompanying clients to medical appointments, arranging necessary follow-up appointments, and collaborating with Staff Supervisors and the nursing staff.
- Coordinating medications-related documentation in accordance with agency policy and MAP regulations.
- Providing crisis intervention and emergency services as needed (including participating in the on-call process as needed).
- Recognizing and reporting concerns related to abuse/neglect, health, and safety.
- Completing all documentation required in the client's electronic health record.
- Completing case management forms and other required documents in a time-sensitive manner.
- Other tasks as required by management
- Associate's Degree (or equivalent) OR High School Diploma / GED AND 3 years of relevant experience (Bachelor's Degree preferred)
- 2 years of experience in human services
- Experience with case management, documentation, and coordinating services in a human service setting
- Strong verbal and written communication skills
