As a Care Coordinator II, you will guide families through the Wraparound process, ensuring each child/youth receives the individualized care and support they need.
Key Responsibilities
- Provide high-quality Wraparound services for children/youth and families.
- Conduct comprehensive assessments, including CANS, CRAFFT (12 ), diagnostic assessments, and Serious Emotional Disturbance determinations.
- Convene and facilitate Care Planning Team (CPT) meetings to develop and update Individualized Care Plans (ICP).
- Maintain consistent communication with youth, families, and care teams to identify strengths, needs, and effective strategies.
- Support youth and family participation in goal setting and implementation.
- Identify and connect families with medically necessary services.
- Collaborate with state agencies, schools, healthcare providers, and community resources.
- Develop transition plans when goals are met.
- Obtain insurance authorizations as required by Managed Care Entities.
- Maintain service productivity and caseload expectations.
- Ensure timely documentation and completion of Risk Management/Safety Plans.

- Master’s Degree in a Human Services-related field (license-eligible).
- CANS-MA certification (required).
- Knowledge of mental illness/SED in children/adolescents and treatment techniques.
- Familiarity with internet, word processing, and email systems.
- Experience navigating child/family-serving systems (preferred).
- Knowledge of community resources (preferred).
- Experience facilitating meetings or groups (preferred).
- Demonstrated cultural competence and ability to work with diverse populations.
- Strong organizational, communication, and facilitation skills
