Position: Care Coordinator (Full-Time | $23-$25/hr | 35 hrs/week)
Location: Holyoke, MA (Hybrid up to 2 days per week after completion of first 6 months)

What You’ll Be Doing:
✅ Support members during transitions between hospitals, rehab centers, and home
✅ Connect members to various resources in the community
✅ Develop, monitor, and adjust individualized care plans
✅ Provide health and wellness coaching to improve long-term outcomes
✅ Conduct in-person visits to homes and community locations as needed
✅ Conduct initial Outreach & Assessments on a regular basis
✅ Build strong connections with local service providers and partners
✅ Champion culturally competent, person-centered care
What You Bring to the Table
✅ Bilingual (Spanish/English) – Required
✅ HS diploma with 4 years of relevant experience or Bachelor’s degree in a human services-related field with at least 1 year of experience
✅ Excellent communication, problem-solving, and organizational skills
✅ Tech savvy – comfortable using email, databases, and electronic records
✅ Valid driver’s license and reliable transportation (local travel required)
✅ Satisfactory CORI, SORI, OFAC, SAM, & OIG background check required.
