Available Schedules
Full Time: Monday-Friday 8:00am-4:30pm
Pay Rate
$21.00-$24.00/hr based on education, experience, credentials, etc.

Your experience, knowledge and great skills will support the Hope Center program with the following:
- Supporting the growth, independence, and safety of individuals served at all times.
- Using an understanding of the assessed needs of individuals served to facilitate referrals to community-based support.
- Researching & communicating access to the continuum of care, including primary care, housing, etc., to individuals served.
- Collaborating with the individual’s family, BHN treatment team & other agencies to ensure successful referrals to & engagement in next steps in the continuum of care.
- Providing information, referrals, and follow-up as specified in the treatment plan of the individuals served.
- Completing transition plan and required documentation in a timely and thorough manner.
- Gathering information and completing billing enrollments.
- Facilitating diverse treatment groups, as needed.

- High School Diploma or GED equivalent (required).
- At least 1 year of experience working with substance use disorders (SUD)/addiction recovery (required).
- Working knowledge of ASAM (Addiction Society of Addiction Medicine) levels of care criteria as well as internal and external resources available to individuals served (required).
- Familiarity with DPH BSAS regulations and community referral sources (preferred).
- Excellent verbal and written communication skills.
- Familiarity with internet, word processing, and email systems.
- Valid driver’s license & car available for work purposes (required).
