Your Responsibilities
- Develop and implement individualized plan-of-care interventions for critically ill patients.
- Collaborate with the interdisciplinary team to coordinate patient care, including transfer, discharge, referral, and spiritual/psychosocial support needs.
- Evaluate learning needs of patients and/or family members and provide education appropriate to age, culture, condition, and circumstances.
