Job purpose
Overall direction, coordination and supervision of the Community Wellness Program in accordance with the Community Policies and Procedures and in compliance with all state, local and federal rules and regulations.
Accountable for the day-to-day supervision of all staff assigned to you. Supervision to include oversight of each assigned staff member’s job performance, including but not limited to, continuous, predictable, quality, safe and efficient care to Community residents.
Coordinate and communicate with all other Community staff in order to provide person centered care to each Community resident. You are expected to lead by example to make certain that the programs and services offered and delivered by you and your staff maximize each resident’s quality of life.
Report all emergency situations, reportable incidents, staffing issues and observable changes in resident behavior and/or health status to the Community Administrator.

The Wellness Director is responsible for overall operation of the Wellness Department. Day to day responsibilities include the following:
- Oversee and ensure accurate and complete documentation of admission and all subsequent assessments and care plans of all current residents.
- Assist with creating protocols, polices, procedures, and philosophy for the community.
- Monitor the implementation of current policies and procedures, which reflect adherence to corporate and external regulatory guidelines.
- Monitor the efficient and effective use of all Wellness department resources within the established and approved budget and staffing levels.
- Ensure that all staff follow established safety regulations, including fire protection, infection control, bloodborne pathogens, hazardous waste, and quality assurance procedures.
- Complete audits of wellness department, including medication carts and electronic health records.
- Supervise the timely and accurate completion of all documentation required for monthly billings for resident services, ancillary charges and changes in resident care.
- Review and follow-up on community incident reports that are health or injury related and develop/monitor plans of correction in accordance with community policies and procedures.
- Perform continuous, ongoing supervision of Health and Wellness team members, including but not limited to training, disciplining, counseling, motivating, and evaluating.
- Participate in regulatory inspections and surveys. Supports and follows up in identifying trends and assists in the development of corrective action plans and troubleshooting resolutions.
- Serve as liaison with residents and family members when needed.
- Assist when necessary in performing initial resident assessments in an effort to expedite resident move in process.
- Participate in community Quality Management Program (QMP) committee.
- Conduct Coordination of Care plan conferences with each Resident and Responsible family members.
- Be available to promptly discuss with prospective residents the facilities and services of the Community and Wellness department.
- Present a professional demeanor that communicates the Community philosophy of care and services to current and prospective residents.
- Handle all resident, family and staff concerns and complaints in a polite, caring and professional manner.
- Maintain current tasks lists for caregivers and update ADLs in a timely manner as changes occur
- with residents.
- Order Wellness department supplies as well as durable medical equipment as needed.
- Assure home health, hospice and ancillary care providers, communicate their visits and provide necessary documentation of their visit.
- Work directly with and cooperate with outside healthcare practitioners, consultants, physicians and all other outside providers in the coordination of resident care.
- Performs on-call duties on a rotating basis with other Wellness department managers.
- Fills-in for the QMAP and/or Caregiver as needed.
- Works as manager of the day as directed by Executive Director.
- Other duties as assigned by the Executive Director.
Qualifications and Experience
- Licensed Practical Nurse or Registered Nurse
- Minimum High School Diploma or GED
- Eager to Learn and to be a Part of a Strong Team
- Pass Background Check and Drug Tests
- Pass Routine TB Test
- Able to use general office equipment and computer systems required.
- Must demonstrate dependable, regular attendance
- Must be able to communicate and function productively on an interdisciplinary team
- Must be able to sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours
- Has familiarity and willingness to work with elderly population
- Receives and maintains CPR Instructor certification
- Maintains unrestricted and active Colorado nursing license

Working conditions
May be exposed to infectious waste, diseases, conditions, etc. Please wear Personal Protective Equipment (PPE) provided.
Physical requirements
Must be able to meet physical demands of lifting and moving over 30 pounds, sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours. Moving at a high pace for long periods of time.
Hours and Dress Code
Exempt, salaried position. Must be available to work 40 hours weekly, including nights and weekends.
