The Resident Care Coordinator is responsible for overseeing the provision of care and services to residents in a senior living community. They ensure that the residents receive high-quality care and services that are customized to their individual needs. They also coordinate with other healthcare professionals, family members, and staff to ensure that the residents receive appropriate care.
Delegated responsibility to perform duties as stated in The Community Policies and Procedures and in compliance with all relevant rules and regulations.
Duties and responsibilities include, but are not limited to:
- Develop and implement individual care plans for residents in the community
- Coordinate with healthcare professionals to ensure that residents receive appropriate medical care
- Communicate with family members and loved ones to keep them updated on the residents’ care and progress
- Supervise and train care staff to ensure that they provide high-quality care
- Conduct regular assessments of residents to determine their changing needs and adjust care plans accordingly
- Maintain accurate and up-to-date records of resident care
- Ensure that the community's policies and procedures are followed to provide safe and effective care to residents
- Monitor medication management and ensure that residents receive their medications as prescribed
- Coordinate with other departments to ensure that all resident needs are met
- Provide support and guidance to residents and their families as they navigate the healthcare system
Qualifications and Experience
· Minimum High School Diploma or GED preferred
- Excellent communication and interpersonal skills
- Ability to work collaboratively with a diverse team of healthcare professionals
- Knowledge of regulations and best practices related to senior care
- Strong organizational skills and attention to detail
- Ability to prioritize and manage multiple tasks effectively

Working conditions
May be exposed to infectious waste, diseases, conditions, etc. Please wear Personal Protective Equipment (PPE) provided. The Resident Care Coordinator works in a senior living community and may be required to work evenings, weekends, and holidays as needed. This position may require standing or walking for extended periods of time and may involve some physical tasks such as lifting or assisting residents. The work environment may be fast-paced and require the ability to respond quickly to emergencies or changing resident needs.

Physical requirements
Must be able to meet physical demands of lifting and moving over 30 pounds, sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours. Moving at a high pace for long periods of time.
Hours and Dress Code
Non-exempt, hourly position. Must be available on weekends.
