Overview
We are seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to join our Home Health and Hospice team. This role involves providing comprehensive social work services to patients across various settings, including home health, hospice, nursing homes, and assisted living facilities. The ideal candidate will have a strong background in behavioral health, clinical counseling, and case management, with a focus on delivering patient-centered care that promotes dignity and quality of life. Experience with medical documentation, and HIPAA compliance. Responsibilities
- Conduct thorough patient assessments to evaluate psychosocial needs and develop individualized care plans.
- Provide clinical counseling services including individual and group therapy for patients dealing with mental health issues, addiction, dementia, PTSD, or developmental disabilities.
- Deliver specialized hospice care support focusing on end-of-life planning, grief counseling, and emotional support for patients and families.
- Coordinate with healthcare teams to ensure seamless delivery of home health services while maintaining compliance with HIPAA regulations and medical documentation standards.
- Assist with intake procedures and facilitate referrals for additional services such as special education or community resources.
- Utilize EMR systems for accurate documentation of patient interactions, progress notes, and treatment plans.
- Engage in case management activities including coordinating services across multiple providers and advocating for patient needs within the healthcare system.
- Master's degree in Social Work from an accredited institution; licensure as a Licensed Master Social Worker (LMSW).
- Proven experience in hospice care and home health settings preferred
- Familiarity with EMR systems and medical records management
- Knowledge of HIPAA regulations and medical documentation standards required to ensure patient confidentiality and compliance.
- Ability to work independently while collaborating effectively within multidisciplinary teams to deliver holistic patient care.
