Bay Home Health and Hospice, a well-established Home Care and Hospice Agency located in Romeo, MI is seeking an Hospice Registered Nurse On Call (RN) to join our Hospice Team, working On call 14 days a month. This position will cover on call mainly in Macomb County, but may need to travel to other parts of the service area to include St. Clair county, Southern Lapeer county, Eastern Oakland county and the Grosse Pointe and Harper Woods.

Job Responsibilities
Act as a patient advocate
Assess and treat/facilitate intervention of the physical, psychosocial and spiritual needs of patients and families by making home visits as needed for skilled nursing intervention and to provide physical and emotional support to the patient and family in a period of crisis; including direct patient care, physician communication, follow-up communication, and necessary care coordination
Coordinate with clinical management to schedule SOC admission assessment visits; perform admission visits, as assigned by clinical management
Establish, develop, and revise assigned patient's plan of care in collaboration with, the attending physician and IDT, that addresses the patient's and the patient's family's overall needs
Provide emotional support to patients and families in a manner that fosters trust and comfort to resolution of concerns/needs
Conduct proactive visits – utilize assessment, teaching, observation, and intervention skills; provide effective pain and symptom management to maximize quality of life for patients and families
Conduct visits with patients in a continuous care or general inpatient care for assessment, symptom management, and/or to meet the physical and emotional needs of the patient and family
Conduct visits upon a patient's expiration in private homes or facilities; assist the patient's family or facility staff with contacting required county/state authorities, contacting funeral home/mortuary services, assisting with medication disposal, and/or notifying contracted vendors; provide emotional support to caregivers, families, and loved ones
Respond to phone calls and email communications from patients, family members, caregivers, company answering services, and/or the interdisciplinary team (IDT) in a timely manner
Develop plan of care to be followed by the Home Health Aide, as necessary
Conducts HHA supervisory visits, at a minimum of every fourteen days; document supervisory visits on clinical records; report constructive feedback to the Administrator, as necessary
Triaging patient related calls by determining the needs of the caller and then responding in an appropriate manner either by telephone or with a visit
Respond to referral calls, providing the caller with information regarding hospice care, screening calls for eligibility, taking patient information
Work closely with the IDT to ensure proper patient follow-up; identify need for intervention by other members of the team on an ongoing basis and collaborate in the planning of such care
Maintain open communication with the IDT including reporting changes in patient condition to the managing physician
Collaborate with the IDT when implementing changes to the plan of care; ensure required approvals are obtained and documented; initiate change in level of care as appropriate to meet the needs of the patient and family
Prioritize patient visits; manage time and resources effectively to meet the needs of the patients and families
Educate patient, family, or caregivers on how to provide proper care; provide ongoing support through demonstration and supervision, as necessary or requested
Ensure documentation is completed in a manner which is consistent with company policy and procedure; complete and submit all required documentation, including home visit documentation and patient admission documentation within 24 hours of the end of the shift
Ensure all continuing education and personal/professional development responsibilities are satisfied:
- Maintain personal health status requirement in relation to position

- Provide current personnel file information to the company in timely manner

- Attend in-service programs to fulfill requirements of position and company policies
- Complete required online training in a timely manner

Education and Experience Required
A graduate of an approved school of professional nursing as determined by the Board of Registration in Nursing
Licensed in the state as a registered nurse by the Board of Registration in Nursing
Associate Degree
Possesses and maintains current CPR certification
Complies with accepted professional standards and practice
Maintains current continuing education as required by state licensure
