What You Will Do:
Coordinate the processes of assessment, planning, implementation, and evaluation of patient care in collaboration with patients, families, physicians, and support services.
Conduct comprehensive and ongoing physical and psychosocial assessments, including functional and nutritional screenings. Use evidence-based assessment tools and analytical models to identify patterns and determine nursing diagnoses. Collaborate with interdisciplinary teams to develop individualized plans of care with clearly defined interventions and outcomes. Implement care interventions, including treatments, medications, and patient/family education, ensuring safe and timely delivery. Delegate and coordinate patient care with support services while maintaining oversight and accountability. Evaluate effectiveness of interventions and revise care plans based on reassessments. Actively involve patients and families in care decisions and discharge planning. Maintain thorough documentation and adhere to nursing practice standards, safety regulations, and compliance requirements.
