What You Will Do:
Performs professional social work to facilitate the movement of patients through the continuum of care while ensuring their biopsychosocial needs are met, appropriate to age and clinical population served.
Conduct comprehensive psychosocial assessments, identifying current problems and contributing factors Provide therapeutic interventions including individual, family, and group sessions as ordered Identify and respond to abuse, neglect, and safety risks (including suicidal or homicidal ideation) in accordance with policy Collaborate with interdisciplinary teams to develop and update patient care plans Communicate effectively with patients, families, and healthcare team members to support informed decision-making Monitor patient progress, reassess needs, and update treatment and discharge plans accordingly Facilitate discharge planning by coordinating community resources and referrals Ensure accurate documentation and compliance with regulatory, legal, and organizational standards Support patient education and engagement throughout the care continuum Promote patient safety, confidentiality, and adherence to HIPAA guidelines
