The Wound Care Specialist provides expert care to patients with acute and chronic wounds. This role involves comprehensive assessment, planning, implementation, and evaluation of wound care interventions. The specialist collaborates closely with the interdisciplinary healthcare team to promote optimal patient outcomes and support the overall care plan. Key Responsibilities:
- Conduct comprehensive assessments of patients with wounds, including evaluation of wound type, size, location, and overall health status.
- Identify and document patient history, including previous treatments, medications, and comorbidities that may affect wound healing.
- Develop and implement individualized treatment plans utilizing evidence-based wound care practices.
- Perform wound care procedures such as cleansing, debridement, dressing changes, and application of advanced therapies.
- Monitor wound healing progress, reassess regularly, and modify treatment plans as clinically indicated.
- Educate patients, families, and caregivers on wound care techniques, prevention strategies, and follow-up expectations.
- Collaborate with physicians, physician assistants, nurses, therapists, and other members of the healthcare team to ensure coordinated, high-quality care.
- Accurately and promptly document all assessments, interventions, and outcomes in the electronic health record.
- Participate in quality improvement initiatives and maintain up-to-date knowledge of best practices in wound care.
- Assist in managing general surgery patients in an outpatient clinic setting and schedule procedures with the operating room (OR) department.
- Coordinate and complete prior authorizations for procedures and diagnostic testing.
- Assist the physician assistant or surgeon with minor in-office procedures.
- Perform other related duties as assigned to support patient care and clinic operations.
- Current state license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
- Minimum of 2 years of clinical experience in wound care or a related healthcare setting.
- Wound care experience preferred; wound care certification a plus but not required.
- Strong clinical assessment, critical thinking, and problem-solving abilities.
- Excellent communication, patient education, and interpersonal skills.
- Ability to work both independently and collaboratively within a multidisciplinary team.
- Proficiency in documentation and use of electronic health record systems.
