Balanced Wellbeing LLC is setting the standard in Florida for Mental Health Services. Our goal is to provide access to a "Dementia Guide" program to caregivers of patients with dementia in skilled nursing and/or assisted living facilities.
Overview:
We are seeking dedicated, organized, and phone-friendly candidates to join our team. As a Dementia Guide Care Navigator, you will be you'll be the first point of contact for caregivers, helping them navigate dementia-related challenges, access resources, and feel supported throughout their caregiving journey.
This crucial role enrolls caregivers into our program, providing emotional support, and guiding them to services tailored to their needs.
Duties:
- Identify and engage eligible caregivers for enrollment into the Dementia Guide Program
- Conduct eligibility screenings and explain program services
- Obtain informed consent and complete intake assessments
- Serve as a consistent point of contact and support for enrolled caregivers
- Connect caregivers with dementia-specific resources, education, and services
- Maintain accurate and timely documentation in our care navigation system
- Collaborate with clinical teams and community partners to support caregiver needs
- Participate in team meetings, trainings, and quality improvement efforts
- Able to remain adaptable and flexible as this program continues to evolve and grow.
Qualifications:
Required:
- 6 months of experience in healthcare, social services, or caregiver support
- Strong communication, empathy, and listening skills
- Ability to build trust and rapport with caregivers from diverse backgrounds
- Proficient in using computers and electronic documentation systems
- Highly organized and self-motivated
- Committed to completing the Dementia Guide training and achieving certification through Balanced Well-Being.
- Confident in conducting phone-based outreach to enroll qualifying candidates into the program.
Preferred:
- Experience working with older adults or individuals living with dementia
- Familiarity with community-based aging or dementia services
