What are the responsibilities and job description for the Health Care Navigator position at BAKERRIPLEY?
Reporting to the Veteran Services Director, the Health Care Navigator is responsible for connecting SSFV enrolled Veterans to health care and mental health care services at the Houston VA Michael DeBakey Medical Center and regional VA Clinics. If Veteran is not eligible for VA Health Administration services, the connection and referral will be made to any other local health care systems servicing the general public. The Health Care Navigator will work closely with agency team members and partnering organizations promoting the overall wellbeing of clients in care.

Health Care Navigator will work closely with agency team members and partnering organizations promoting
the overall wellbeing of clients in care.
1. Health Care Navigator (HCN) in collaboration with VAMC or community health care
providers, supports designated veterans’ health plans to identify barriers and challenges to
accessing health care services.
2. Assists the Veteran identify concerns and questions and shares them with the health care
team; reminds Veteran of providers’ recommendations to ensure he/she has a clear
understanding of proposed treatment options.
3. Provides linkages to resources (i.e., medical studies, trials & treatments) and potential
benefits (i.e., SSI/SSDI/TANF, SOAR, Montgomery GI Bill, MHMRA, etc.) for enrolled Veterans
and their families.
4. Manages client files safely and follows safety protocols and procedures to preserve the
Personal Identifiable Information of all clients in care.
5. Exhibits a high level of professionalism while advocating for the veteran and representing
BakerRipley and the Veteran Services team during internal and external meetings and
program-related events.
Experience
Experience in clinical settings, critical care or substance disorders or recovery a plus.
Experience in clinical, medical or human services environments.
Advanced education or equivalent work experience preferred.
