Key Responsibilities
- Patient & Family Support: Assess psychosocial needs of patients and families; provide counseling, crisis intervention, and emotional support.
- Care Coordination: Collaborate with nurses, physicians, and the interdisciplinary team to develop personalized care plans.
- Cultural Sensitivity: Identify cultural needs and educate the care team on respectful, inclusive practices.
- Resource & Education: Connect patients and families to community, financial, and legal resources; assist with advance directives and end-of-life planning.
- Documentation & Compliance: Maintain accurate, timely clinical records per professional and organizational standards.
- Collaboration & Advocacy: Serve as liaison between families, caregivers, and community agencies to ensure seamless, patient-centered support.
