- Conduct assessments, gather data, and document resident information accurately and thoroughly
- Collaborate with healthcare professionals to identify and address resident needs
- Develop and maintain relationships with residents, families, and staff to promote open communication and collaboration
- Participate in quality improvement initiatives to enhance resident care and satisfaction
- Maintain accurate and up-to-date records and reports
- Current registered nurse (RN) license in the state of Illinois
- Experience in long-term care or post-acute care setting preferred
