Overview
Greenville Nursing and Rehabilitation is seeking an experienced and detail-oriented Registered Nurse to join our leadership team as an MDS Coordinator.
This role is ideal for an RN who is passionate about clinical excellence, regulatory compliance, quality outcomes, and ensuring residents receive the highest standard of care.
As the MDS Coordinator, you will oversee the MDS process, support accurate reimbursement, and collaborate closely with interdisciplinary teams to ensure resident-centered care and compliance with federal and state guidelines.

Schedule
- Full-Time Position
- Monday–Friday schedule
- On-call rotation required for clinical leadership support
- Additional hours as needed to support resident care and regulatory requirements
Key Responsibilities
- Coordinate and oversee all MDS assessments and submissions
- Ensure compliance with federal, state, and facility regulations
- Monitor assessment schedules and submission deadlines
- Collaborate with nursing, therapy, dietary, and social services teams
- Develop and update individualized resident care plans
- Support quality measures and reimbursement accuracy initiatives
- Review clinical documentation for accuracy and completeness
- Assist with survey readiness and regulatory compliance efforts
- Monitor changes in resident condition and ensure timely assessments
- Serve as a resource for staff regarding MDS and documentation processes
Qualifications
- Active Illinois Registered Nurse (RN) license in good standing
- Long-term care experience preferred
- Prior MDS experience strongly preferred
- Strong understanding of Medicare, Medicaid, and regulatory requirements
- Excellent organizational and time-management skills
- Strong communication and leadership abilities
- Ability to work independently and collaboratively with interdisciplinary teams
- Proficiency with EMR and clinical documentation systems
