Resident Care & Clinical Oversight
- Conduct and oversee initial and ongoing resident health assessments and service plans.
- Monitor changes in resident conditions and ensure appropriate clinical interventions are implemented.
- Administer medications and treatments per physician orders and in accordance with state regulations.
- Coordinate and supervise care provided by nursing assistants and caregivers.
- Maintain accurate and timely documentation of resident care, assessments, and communication logs.
- Provide leadership, training, and support to care staff to ensure consistent delivery of quality care.
- Schedule, supervise, and evaluate care team performance, promoting teamwork and accountability.
- Assist with recruitment, onboarding, and retention of care staff.
- Ensure compliance with all regulatory, licensing, and safety requirements.
- Participate in audits, quality assurance initiatives, and resident care conferences.
- Maintain confidentiality and adhere to HIPAA guidelines.
- Serve as a primary point of contact for residents and families regarding health concerns and care plans.
- Support residents’ rights, dignity, and individuality in all aspects of care.
- Communicate effectively with families, physicians, and interdisciplinary teams.
- Maintain accurate resident records, medication administration records (MARs), and physician orders.
- Participate in community meetings, safety drills, and staff training programs.
- Assist the Health Services Director with reporting and compliance documentation.
Qualifications
- Education: Graduate of an accredited nursing program; current state LPN license in good standing.
- Experience: Minimum 1–2 years of nursing experience, preferably in assisted living, long-term care, or geriatric settings.
- Skills:
- Strong clinical assessment and critical-thinking abilities
- Excellent communication and leadership skills
- Ability to multitask, prioritize, and manage time effectively
- Compassionate, patient-centered approach to care
- Certifications: CPR and First Aid certification (or ability to obtain upon hire).
