Position Summary
The MDS Nurse (RN/LPN) is responsible for coordinating and completing the Minimum Data Set (MDS) assessments in accordance with federal and state regulations. This role ensures accuracy in documentation, supports care planning, and collaborates with interdisciplinary teams to optimize resident outcomes and reimbursement.
Key Responsibilities
Complete and coordinate MDS assessments (OBRA, Medicare, and PPS) accurately and on time
Ensure compliance with all CMS and state regulations
Develop, review, and update individualized care plans
Collaborate with nursing staff, therapy, and other departments
Monitor documentation to support appropriate reimbursement
Participate in audits and quality assurance processes
Educate staff on MDS processes and documentation requirements
Track and manage assessment schedules and deadlines
Qualifications
For RN:
Active Registered Nurse (RN) license in the state
Previous MDS experience preferred
Strong knowledge of CMS guidelines and reimbursement systems
For LPN:
Active Licensed Practical Nurse (LPN) license in the state
MDS experience preferred (training may be provided)
Understanding of long-term care regulations
For Both:
Experience in long-term care or skilled nursing facility preferred
Strong attention to detail and organizational skills
Excellent communication and teamwork abilities
Proficiency in electronic medical records (EMR) systems
