- Conduct thorough patient assessments, including vital signs, physical examinations, and comprehensive health histories
- Administer medications via IV infusion, injections, and other routes following strict aseptic techniques
- Provide specialized care such as wound suturing, ostomy care, and tube feeding management
- Manage complex medical conditions including dialysis support, cardiac catheterization recovery, and acute pain management
- Perform diagnostic evaluations and coordinate care with physicians using EMR (Electronic Medical Record) systems like Epic or Cerner
- Educate patients and families on disease management, medication administration, and health coaching strategies
- Assist with triage assessments for urgent needs and coordinate appropriate interventions in home or community settings
- Ensure compliance with HIPAA regulations by maintaining accurate medical documentation and patient records
- Support patients with developmental disabilities, memory care needs, or those requiring hospice & palliative medicine services
- Utilize advanced skills in infection control, laboratory specimen collection/processing, and ventilator management as needed
- Valid RN or LPN license in good standing with relevant state licensing boards
- Proven experience working in homecare settings with diverse patient populations including geriatrics, pediatrics, or individuals with disabilities
- Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks
- Strong knowledge of medical terminology, anatomy & physiology, ICD-10/ICD-9 coding standards and CPT coding procedures
- Experience with IV insertion, catheterization (urinary or vascular), phlebotomy and specimen collection/processing
- Critical care experience including ICU or emergency room background highly preferred
- Ability to perform complex procedures such as suturing, ostomy care, NG tube management and ventilator support
- Excellent communication skills for effective patient assessment and interdisciplinary collaboration
- Knowledge of Workers' Compensation law and familiarity with healthcare regulations related to home health services
- Demonstrated ability to work independently while managing multiple patient cases efficiently
