Key Responsibilities
MDS & Care Plan Coordination
- Coordinate and complete MDS assessments in accordance with CMS guidelines and required timeframes.
- Ensure the accuracy and completeness of resident assessment data.
- Facilitate Care Area Assessments (CAAs) and participate in developing individualized resident care plans.
- Schedule and track all required assessments and submissions.
- Monitor assessment schedules to ensure timely completion and transmission.
- Maintain compliance with federal, state, and facility regulations related to the MDS process.
- Ensure accurate coding to support quality measures and reimbursement.
- Review clinical documentation to validate MDS coding and identify opportunities for improvement.
- Stay current on changes to CMS regulations, reimbursement systems, and industry best practices.
- Assist with survey preparation and participate in regulatory audits.
- Work closely with nursing staff, therapy departments, social services, dietary, and other interdisciplinary team members.
- Participate in resident care conferences and interdisciplinary meetings.
- Educate staff regarding documentation requirements and MDS-related processes.
- Collaborate with clinical leadership to improve resident outcomes and quality indicators.
- Monitor quality measures and identify trends affecting facility performance.
- Assist in developing and implementing performance improvement initiatives.
- Support efforts to enhance resident satisfaction and quality of care.
Education
- Graduate of an accredited School of Nursing.
- BSN preferred.
- Current and active Registered Nurse (RN) license in the state of employment.
- Minimum of 1–3 years of nursing experience in a skilled nursing facility or long-term care setting.
- Previous MDS experience preferred.
- Experience with Medicare, Medicaid, and CMS regulations strongly preferred.
- Thorough knowledge of MDS 3.0, RAI process, and care planning requirements.
- Understanding of PDPM, quality measures, and reimbursement systems.
- Strong assessment, analytical, and critical-thinking skills.
- Excellent organizational and time-management abilities.
- Effective communication and collaboration skills.
- Proficiency in electronic medical records (EMR) systems and MDS software.
