Key Job Functions:

• Provide emotional support to residents and families. 
• Assist residents and families with direct payee applications, Medicaid applications, eligibility guidance, and required
  documentation.
• Assess resident needs, develop care plan recommendations, coordinate interventions, and work collaboratively with interdisciplinary teams. 
• Build and maintain relationships with community agencies and organizations to expand support services available to residents. 
• Maintain accurate resident records, documentation, reports, and community resource lists in accordance with confidentiality and legal requirements. 
• Oversee discharge planning for long-term care residents and coordinate appropriate community resources and services. 
• Collaborate with administrators, billing, healthcare professionals, residents, and families to coordinate services and advocate for resident needs and preferences. 
• Provide emotional support to residents and families dealing with grief, loss, adjustment, coping challenges, and long-term care transitions. 
• Conduct assessments of residents’ social, emotional, and psychological needs and contribute to care planning and MDS documentation. 
• Submit Level of Care applications and supporting documentation for residents requiring nursing home level of care. 
• Gather and upload admission information into the electronic health record, including PASS documentation, contact information, insurance cards, payment planning information, and resident discharge or long-term care intentions.
