We are seeking a dedicated Physical Therapist to provide outpatient rehabilitation services primarily in-home for older adults, including private residences and assisted living facilities.

- Assessing patient needs and developing personalized therapy programs to restore and enhance function
- Educating patients and caregivers on skills to promote independence and safety
- Referring patients to other disciplines as necessary and collaborating with the rehabilitation team
- Supervising support personnel, adhering to documentation and scheduling standards
- Performing evaluations, treatments, and modifications to therapy plans based on patient progress
- Ordering supplies, maintaining equipment, and ensuring a safe environment
- Designing and instructing on the use of adaptive devices and mobility techniques
- Incorporating health and wellness programs, fall prevention, and community mobility assessments
- Valid Physical Therapy license in applicable state(s) or proof of eligibility
- Master’s degree in Physical Therapy or equivalent, or a Bachelor’s plus five years of experience
- Reliable transportation or valid driver’s license
