Annual Salary: $120k-$160k (varies depending on years of experience)
We are seeking a dedicated and compassionate Family Nurse Practitioner to join a dynamic healthcare team focused on delivering high-quality, patient-centered care. This role involves managing a diverse adult patient population and contributing to wellness initiatives within the community.
Key Responsibilities:
- Conduct thorough assessments and manage both acute and chronic health conditions.
- Initiate and oversee treatment plans for mental health concerns such as anxiety and depression.
- Prescribe medications and therapies in accordance with clinical guidelines and licensure scope.
- Order and interpret diagnostic tests, applying evidence-based practices and cost-effective care principles.
- Educate patients on preventive health measures, lifestyle changes, and treatment options.
- Maintain detailed and accurate documentation in the electronic medical record system.
- Participate in quality improvement efforts and peer review processes.
- Collaborate with a multidisciplinary team to ensure coordinated and comprehensive care.
- Engage in outreach and health education activities to support community wellness.
- Refer patients to specialists or external services when clinically indicated.
- Take part in pre-visit planning and care coordination with other team members.
- Attend team meetings and contribute to clinic-wide initiatives.
- Provide care to a culturally diverse patient population, including individuals new to the country.
- Share responsibility for after-hours and weekend on-call coverage.
- Ensure compliance with all relevant healthcare regulations, including HIPAA and mandatory reporting.
- Maintain current certifications, licensure, and DEA registration.
- Participate in ongoing professional development and continuing education.
Requirements:
- MSN (Master of Science in Nursing), DNP (Doctor of Nursing Practice), or equivalent advanced degree.
- Completion of an accredited Nurse Practitioner program.
- Board certification as a Family Nurse Practitioner.
- Active and unrestricted license to practice in New York State.
- Strong interpersonal and communication skills.
- Additional certifications in HIV care or LGBTQ health are a plus.
