Responsibilities

• Assist residents or patients with personal care tasks such as bathing, dressing, grooming, and mobility assistance in accordance with care plans.
• Monitor and record vital signs including blood pressure, pulse, temperature, and respiration rates accurately using Electronic Health Record (EHR) systems.
• Support medication administration under the supervision of nursing staff while adhering to HIPAA regulations and maintaining strict confidentiality of patient information.
• Provide specialized care for residents with Alzheimer’s disease, dementia, or memory impairments, including engaging activities and ensuring safety protocols are followed.
• Perform basic medical procedures such as catheter care, tube feeding assistance, wound dressing changes, and first aid when necessary.
• Document all patient interactions and medical records precisely using EMR (Electronic Medical Records) systems like Epic or Cerner.
• Assist with mobility and heavy lifting tasks safely using manual handling techniques to prevent injury for both staff and residents.
• Maintain a clean and safe environment by following infection control practices aligned with state healthcare regulations.
• Communicate effectively with residents, families, and healthcare team members to ensure coordinated care delivery.
• Support hospice or home care services by providing compassionate assistance tailored to individual needs.

Qualifications

• Prior experience working in assisted living facilities, nursing homes, hospitals, or senior living environments is highly preferred.
• Demonstrated experience working with individuals with developmental disabilities or in memory care settings.
• Knowledge of physiology, anatomy, medical terminology, and basic nursing practices required for patient care.
• Certification as a CNA (Certified Nursing Assistant) or equivalent license recognized by the State Board of Nursing is mandatory.
• Valid CPR and First Aid certifications; additional certifications in Alzheimer's or dementia care are advantageous.
• Familiarity with EMR/EHR systems such as Epic or Cerner; experience with home & community care settings is a plus.
• Ability to perform physically demanding tasks including heavy lifting and manual handling while maintaining safety standards.
• Strong communication skills to effectively collaborate within multidisciplinary teams and provide excellent caregiver communication.
• Knowledge of HIPAA regulations and commitment to maintaining patient confidentiality at all times.
• Understanding of state healthcare regulations governing senior care and medical documentation requirements.

Pay: $23.00 - $26.00 per hour
