Duties

  • Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner.
  • Administer medications and injections accurately following established protocols, including IV infusions and medication management for pain control or other medical needs.
  • Assist with diagnostic evaluations such as specimen collection, laboratory testing, and medical imaging procedures while maintaining strict infection control standards.
  • Provide wound care, catheterization, tube feeding management, and sterile technique to ensure optimal patient outcomes.
  • Support patients with activities of daily living (ADLs), including mobility assistance with Hoyer lifts and ensuring safety during transfers or ambulation.
  • Collaborate with multidisciplinary teams in acute care environments like ICU or emergency rooms to deliver timely triage and critical interventions.
  • Document all patient interactions thoroughly in compliance with HIPAA regulations and nursing documentation standards.
  • Educate patients and their families on health management strategies, including dementia or Alzheimer's care, hospice support, or chronic disease management.
  • Participate in discharge planning, case management activities, and long-term care regulations to facilitate smooth transitions across care settings.

Experience

  • Valid state licensure as a Licensed Practical Nurse with current CPR/BLS certification.
  • Proven experience working in diverse healthcare environments such as hospitals (including Level I Trauma Centers), nursing homes, skilled nursing facilities, assisted living communities, or home care settings.
  • Demonstrated proficiency in EMR/EHR systems like Epic, Cerner, Athenahealth, or eClinicalWorks for accurate medical documentation.
  • Critical care experience in ICU or emergency room settings is highly desirable; familiarity with Level II trauma center protocols is a plus.
  • Experience working with geriatric patients or individuals with disabilities such as developmental delays or dementia enhances your ability to provide specialized memory care and behavioral health support.
  • Knowledge of ICD-9/ICD-10 coding for accurate diagnosis documentation; experience with CPT coding is advantageous for billing processes.
  • Skilled in IV insertion, phlebotomy, specimen processing, and basic life support techniques to ensure comprehensive patient care delivery.
  • Strong understanding of infection control practices and long-term care regulations to maintain compliance and safety standards across all clinical activities.
