AtlantiCare’s primary care practices are advancing the patient-centered medical home model by offering same- or next-day appointments, robust EMR utilization, and care coordination support for complex patients. Our physicians benefit from a collaborative relationship with AtlantiCare hospitalists who manage inpatient care at our two medical centers.
