Position Summary
The State Hospital Discharge Initiative Case Coordinator serves as a Behavioral Health consultant supporting older adults transitioning from state psychiatric hospitals to licensed Assisted Living Facilities (ALFs). This role focuses on care coordination, compliance monitoring, and ensuring safe and effective transitions into community-based settings.

Position Summary

Key Responsibilities
- Coordinate referrals and admissions to Assisted Living Facilities (ALFs) for individuals discharging or diverting from state hospitals
- Conduct virtual and in-person care coordination visits to monitor health, safety, and behavioral health needs
- Support care transitions between hospitals, ALFs, and community-based settings
- Monitor participating ALFs for compliance with federal Home and Community-Based Services (HCBS) requirements and state licensing standards
- Conduct annual audits and complete required HCBS documentation and reporting
- Bachelor’s degree in Social Work, Psychology, Public Health, or a related human services field
- Minimum of 3 years of relevant experience in behavioral health, case management, public health, or social services (or 5 years of relevant experience in lieu of degree)
- Strong experience in care coordination, community outreach, or advocacy
- Ability to manage multiple priorities in a fast-paced environment
- Knowledge of behavioral health systems and long-term care services
- Experience working with older adult populations
- Strong communication, organizational, and problem-solving skills
- Proficiency in Microsoft Office (Outlook, Teams, Excel, Word)
- Ability to work independently and collaboratively across multidisciplinary teams
- Travel required within the state of Maryland
- Position requires background check
- Valid driver’s license preferred
