Essential Duties And Responsibilities

- Performs at the expert level in directly providing, coordinating, and monitoring professional social work and behavioral health case management services to children, youth, adults with behavioral health needs, and their families.
- Engages clients and families impacted by violence, criminal behavior, and behavioral health challenges to assess needs, strengths, risks, and readiness for change using trauma-informed and culturally responsive approaches.
- Develops, implements, and updates comprehensive, client-centered service and safety plans to address abuse, neglect, dependency, exploitation, family dysfunction, criminal involvement, and other complex psychosocial issues.
- Coordinates and links clients to behavioral health treatment, medical care, housing, employment, education, victim services, legal supports, and other community resources; monitors progress and service effectiveness.
- Collaborates closely with law enforcement, courts, community violence intervention (GVI), behavioral health providers, and other community partners to support risk reduction and long-term stability.
- Provides intensive case management, advocacy, and crisis intervention to promote safety, reduce recidivism, and support recovery and stabilization.
- Conducts regular home, community, and office visits, documents all contacts thoroughly, and maintains timely, accurate, and confidential electronic and paper case records.
- Participates in multidisciplinary team meetings, case staffing, and case conferences to review progress, address barriers, and adjust service strategies.
- Educates clients and families on available services, program expectations, rights, and responsibilities, and supports them in navigating complex service systems.
- Ensures compliance with all applicable federal, state, local, and agency policies, procedures, regulations, and documentation standards.
- Additional duties as assigned.
- Excellent organizational skills and attention to detail.
- Ability to work independently; must be self-motivated and able to manage a complex caseload and competing priorities.
- Excellent verbal, written, and listening communication skills, including the ability to engage individuals and families in crisis.
- Ability to establish and maintain effective working relationships with clients, families, coworkers, community partners, and justice-system stakeholders.
- Ability to understand and adhere to the duties, methods, and procedures required by the position, including confidentiality and ethical standards.
- Demonstrated ability to work with diverse populations affected by trauma, violence, substance use, and mental health conditions.
- Bachelor’s degree or higher in Behavioral or Social Science or related field (such as social work, psychology, sociology, counseling, human services, or a closely related discipline), required.
- Three years’ experience in case management, behavioral health, social services, or a related field, including coordinating services for individuals or families with complex needs.
- Experience working with individuals and families impacted by community violence, criminal justice involvement, or high-risk behaviors is strongly preferred.
- Experience with trauma-informed care, motivational interviewing, or evidence-based practices in behavioral health is preferred.
- Prolonged periods of sitting at a desk and working on a computer.
- Standard hours of work Monday through Friday, 9 am – 5:00 pm; however, evening and weekend hours may be required to support program needs and client engagement.
- Ability to travel within the community for home visits, meetings, and collaboration with partners; may require use of personal vehicle in accordance with agency policy.
