Duties
- Conduct thorough patient assessments including vital signs, physical examinations, and detailed medical histories using advanced EMR (Electronic Medical Record) systems such as Epic or eClinicalWorks.
- Develop and implement personalized treatment plans for acute and chronic conditions across multiple specialties including internal medicine, geriatrics, pediatrics, and behavioral health.
- Administer medications via injections, IV infusions, or other routes while adhering to HIPAA regulations and sterile processing techniques.
- Perform diagnostic evaluations utilizing laboratory experience, sonography, or medical imaging tools; interpret results for accurate diagnosis.
- Manage complex cases involving dialysis patients, hospice & palliative medicine needs, or patients requiring specialized procedures like spinal taps or cardiac catheterization.
- Provide telehealth consultations to expand access to care for remote or underserved populations while ensuring compliance with all telemedicine protocols.
- Collaborate with multidisciplinary teams on discharge planning, case management, utilization review, and post-acute care strategies to optimize patient outcomes.
- Educate patients on health coaching topics such as nutrition, medication administration, wound care (including suturing), and disease prevention strategies.
- Maintain meticulous medical documentation including CPT coding for procedures performed and ICD-10 coding for diagnoses to ensure accurate billing and compliance.
Qualifications
- Valid Nurse Practitioner license with current certification in advanced practice nursing; experience working with individuals with disabilities or developmental disabilities is preferred.
- Demonstrated proficiency with EMR/EHR systems such as Epic or Athenahealth; familiarity with Cerner is a plus.
- Extensive clinical experience in diverse settings including hospital medicine, emergency medicine (Level I & II trauma centers), ICU/PICU units, outpatient clinics, or urgent care facilities.
- Strong knowledge of physiology, anatomy, pharmacology (including psychopharmacology), and medical terminology necessary for comprehensive patient assessment and treatment planning.
- Experience performing procedures such as suturing, catheterization (urinary or vascular), phlebotomy/venipuncture, NG tube placement (tube feeding), and sterile technique practices.
- Ability to handle complex cases involving infectious disease care, acute pain management, occupational health issues like Workers' compensation law compliance or triage protocols.
- Excellent communication skills with the ability to educate patients across age groups—including children (toddler care) and elderly populations—while working within HIPAA guidelines.
- Critical care experience including ICU or trauma center exposure is highly desirable; familiarity with ICD-9/ICD-10 coding standards is essential for accurate documentation. Pay: $114,000.00 - $150,000.00 per year
