Responsibilities:
- Coordinate and conduct accurate and timely MDS assessments for residents in our skilled nursing facilities
- Review medical records and conduct resident interviews to gather necessary information
- Provide education and support to our care team to ensure compliance with MDS regulations
- Identify and report concerns or issues related to resident care and quality of life
- Collaborate with other departments to ensure seamless transitions and comprehensive care
- Maintain accurate and up-to-date records and reports
- Bachelor's degree in Nursing or related field
- Experience in a skilled nursing setting, preferably in MDS coordination
- Current licensure as a Registered Nurse in the state of Missouri
- Strong knowledge of MDS regulations, guidelines, and best practices
- Excellent communication, interpersonal, and organizational skills
