Responsibilities:
- Provide direct patient care to residents, including administering medications, performing wound care, and assisting with daily living activities
- Conduct initial patient assessments and develop individualized care plans
- Collaborate with other healthcare professionals to ensure coordination of care and effective communication
- Monitor and report changes in patient condition to the healthcare team
- Participate in quality improvement initiatives to ensure high-quality patient care
- Maintain accurate and thorough records of patient care and treatment
- Current Licensed Practical Nurse (LPN) licensure in the state of Missouri
- Experience as an Licensed Practical Nurse (LPN) in a geriatric setting or related field preferred
