Job Purpose: As a Therapist/Discharge Planner at Aspire Health Partners, your job purpose will be to provide a multitude of essential functions in order to assist psychiatric inpatient clients to achieve and maintain stability.

Job Functions

• Facilitating group and individual therapy.
• Identifying placement needs and supportive linking resources.
• Interviewing clients to aid in completing their psychosocial assessments and developing their treatment plans with an emphasis on immediate discharge goals.
• Efficiently documenting all activities.
• Conducting activities with the client, client families and/or significant others,
• Providing regular input to the treatment plan.
• Providing support and crisis intervention when necessary.
• completing training in order to continuously enhance job performance and showing flexibility and teamwork.
• Most importantly, all functions must be completed while maintaining high standards of ethical and professional conduct while adhering to agency policies and procedures.

Position qualifications: Ability to receive and maintain a(n)

• Bachelor's Degree in Counseling, Social Work, Psychology or other related field
• Master's Degree highly preferred
