Job Summary:
Under the supervision of the Director of Social Services, the Social Services Coordinator’s primary responsibility is to ensure that services are provided in a planned, effective and timely manner to eligible clients. The Coordinator also ensures that the services are appropriate to meet the needs of the clients, and will contribute to self-sufficiency of participating clients. The duties extend to other client support programs that provide a holistic approach towards assisting community residents.
Responsibilities:
Works with client(s) and staff members of other ADSSA programs to strategize an appropriate service plan and address needs related to obtaining holistic results.
Provides direct service support and referrals as needed and collaborates with other programs in areas of housing, employment, job training, education, legal services, counseling, health, and emergency services related to the client(s) service plan.
Provides direct case management services to clients to assist toward the goal of social self-sufficiency.
Conducts outreach throughout rural communities by traveling to designated service areas to increase access to programs and services.
Develops, strengthens, and maintains partnerships with community leaders, organizations, schools, healthcare providers, faith-based organizations, and other local stakeholders to improve coordination of services and expand community resources.
Represents ADSSA at community meetings, outreach events, health fairs, and other public engagement activities to promote agency programs and services.
Travels regularly within the agency's service area to provide direct services, conduct client visits, facilitate referrals, and ensure equitable access for residents in underserved and rural communities.
Understands and manifests the philosophy and mission of ADSSA.
Reports on activities, visits, outreach efforts, and special client concerns to the Director/Manager and other staff during required staff meetings or as needed.
Orders, prepares, packs, organizes, stores, and distributes food, furniture, appliances, clothing, and other commodities to clients. Must be able to move, push, or lift up to 50 lbs.
Assists with the set-up and operation of events associated with programs such as Emergency Food Bank, Clothing Closet, R.A.Y.S. Program, and Senior Food Distribution.
Assists with volunteers.
Fosters a climate that promotes open communication and maintains confidentiality with clients/patients.
Conducts and/or coordinates educational classes.
Maintains knowledge of community resources and actively participates in outreach and partnership development efforts.
Reviews and maintains accurate and up-to-date client files, including intake, assessments, filing, required documentation, and data entry into agency software for each client.
Responsible for protecting the confidentiality of any information or material obtained in service with the organization, including but not limited to client information, services rendered, donor information, internal and external investigations, and financial information.

Personal Protective Equipment:
- Follow Standard Precautions using personal protective equipment as required.
Minimum Qualifications/Education:
- Associates Degree in a Social Service related field or 2 years of experience as a Social Services Coordinator, preferred.
- High School degree or equivalency, required.
- Valid Class C Texas Driver License, required.
- Certified Community Health Worker state certificate, preferred.
- Federal and/or state program experience including application intake, eligibility and submission.
- Must be computer literate.
- Bilingual preferred (English/Spanish)
- Ability and willingness to travel regularly throughout the agency's service area, including rural communities, using a personal or agency vehicle.
- Reliable transportation and proof of automobile insurance, if using a personal vehicle for agency business, required.
