School Based Health Services Nurse

pResponsible for professional, medical treatment of students in the Edna Karr High School DePaul Community Health Center Clinic and other school clinics when needed./ppResponsible for early detection of physical problems and health evaluations/plans./ppResponsible for disease prevention./ppAssists in promoting programs on health-education and immunization./ppMaintains clinical records./ppCoordinates vision, hearing, and orthopedic screening and health assessments./ppAssists teachers in instructing students in health and nutrition education./ppInterviews patients, measures vital signs and records information in the Electronic Medical/ppRecord./ppPerforms select clinical duties including patient services, certain lab procedures, maintains/pppatient’s medical records in an orderly manner, and reports abnormal results of any service to the primary care provider as per protocol. Assists with prior authorizations and medical refills when directed.  /ppPrepares treatment rooms for examination of patients including procedure room and keeps/ppmedical equipment clean and orderly.  Monitors supplies in each examination room./ppMaintains a neat and clean workspace./ppEscorts assigned patients  and fully prepares the patient for the visit which includes taking  vital signs, completing the Health Risk Appraisal, performing in house labs , review charts for accuracy, ensure diagnostic testing results and consultant reports are scanned in the Electronic Medical Record, review health provider orders, administer medications, perform treatments such as but not limited to breathing treatments, appropriately and accurately document the outcome of the treatment or intervention;   assist behavioral health providers, consultants and patients as needed.  /ppAssists with maintaining a clean and orderly environment including cleaning all medical instruments according to Lab and General Safety Manual./ppUse Standard Precautions with all patients, specimens and equipment./ppUse Needlestick Precautions with all patients, specimens and equipment./ppSupport policies, procedures and supervisory decisions with in the Health Center./ppAccept supervision and feedback from administration. Participates in continuing education programs as directed./ppUse established channels of communication and proper lines of authority for problem solving and communication. Be knowledgeable of and apply the Vision, Mission, and Core Values of the DCHC and Ascension health./ppMaintains a professional demeanor including work attire and appearance./ppAssist in documentation of incidents and untoward events and understands; participates in safety programs and disaster/fire delays./ppMaintains an acceptable attendance record; provide proper justification for absences and tardiness; document overtime including justification./ppAttend DCHC general staff meetings, department meetings, and annual retreats./ppIs knowledgeable of patient’s rights and responsibilities and accepts responsibility for the assurance of the same.  Maintain and assure confidentiality in accordance with HIPAA./ppPerforms other related duties as assigned./ppKnowledge of community agencies to help students and their families./ppKnowledge of district policies and procedures./pp id="isPasted" style='margin:0in;margin-bottom:.0001pt;font-size:16px;font-family:"Times New Roman",serif;line-height:150%;'stronguMINIMUM QUALIFICATIONS/u/strong/pul style="list-style-type: disc;margin-left:undefined;"liRegistered nurse RN certification required./liliCurrent nursing license required.   /liliPossess a sense of discipline to work with accepted nursing and medical standards.
