Ascendo Healthcare Staffing is URGENTLY seeking an experienced Home Health Registered Nurse for a lucrative 13-week travel contract in Wausau, Wisconsin.

If you're an experienced traveler with strong Home Health, OASIS, episode management, and care coordination experience, this could be your next great assignment!

POSITION DETAILS

• Position: Home Health Registered Nurse
• Location: Wausau, WI
• Contract: 13 Weeks
• Start: ASAP
• Guaranteed Hours: 40 Hours/Week
• Schedule: 8-Hour Days, approximately 8:00 AM–4:30 PM, with some flexibility based on census
• Weekend/Holiday: Approximately every 5th weekend holiday rotation
• On-Call: Approximately 2–3 times per month
• Mileage: Reimbursed at the applicable government-approved rate
• Scrubs: Navy Blue
• Floating: May occur to Medford, Antigo, or Wisconsin Rapids, although the primary and highest current need is Wausau.

HOME HEALTH CASELOAD & TRAVEL

Manage approximately 20–25 patients, averaging about 4.5 visits per day. Visit volume may be adjusted on higher-travel days.

Daily travel typically ranges from approximately 25–60 miles, depending on territory, census, patient locations, and staffing needs. The team works to minimize unnecessary travel whenever possible.

WHAT WE’RE LOOKING FOR

This position requires an RN who can step into an active Home Health caseload and work independently from the start.

Requirements

• 2 years of Home Health experience,
• Prior travel experience
• OASIS experience
• Active Wisconsin or Compact RN license,
• AHA or American Red Cross BLS,
• OASIS certification is a plus but not required.
• Experience using Epic Remote Client/Remote Care.

CLINICAL RESPONSIBILITIES

You'll complete OASIS admissions and assessments across all applicable time points, manage episodes of care, communicate with physicians, coordinate interdisciplinary services, and maintain timely EMR documentation.

Clinical responsibilities may include wound care, IV therapy, port care/access, lab draws, chronic disease management, medication education, patient assessments, and care coordination.
