Position Summary:

Arthur Center is looking for an Emergency Room Enhancement (ERE) Outreach Coordinator who is skilled, resourceful, and energized by the challenge of reaching people at their most vulnerable moments and helping them find a path forward. In this role, you will coordinate services for the ERE program across our six-county catchment area, processing referrals, completing needs assessments, connecting clients to mental health and community resources, and working alongside emergency service systems to reduce avoidable crisis readmissions. This is active, relationship-driven work that requires clinical skill, deep community knowledge, and the flexibility to meet people wherever they are. Key Tasks And Responsibilities

ERE Program Coordination & Referral Processing

  • Process referrals into the ERE program and complete the Independent Health Needs Assessment (IHNA) with each client.
  • Engage and support clients who frequently interact with emergency services within the catchment area.
  • Assist with the coordination of hospital discharges as needed to ensure smooth transitions of care and prevent readmissions; engage existing providers to ensure continuity of care.
  • Attend agency and community meetings assigned by supervisor.
  • Establish and maintain positive working relationships with local community resources including law enforcement, ambulance personnel, crisis call centers, and hospitals.


Client Support & Resource Navigation

  • Provide client assistance in accessing mental health and medical services.
  • Assist clients with accessing necessary public services including financial assistance, housing, and basic needs for food, shelter, and clothing.
  • Aid clients in accessing social, educational, vocational, health, and housing resources identified as needs for independent functioning.
  • Provide training, coaching, and support in daily living skills including housekeeping, cooking, personal grooming, transportation, budgeting, paying bills, and maintaining independent residence.
  • Provide adequate follow-up with clients regarding appointments, forms, and other necessary actions.
  • Advocate effectively for clients within systems and with service providers.


Crisis Response & Mobile Outreach

  • Assist with daytime mobile crisis response as needed.
  • Participate in crisis on-call duties as scheduled.
  • Engage with clients in a trauma-informed and collaborative manner that supports safety, empowerment, voice, and choice.
  • Respond to community crises in a timely manner and apply appropriate de-escalation and behavioral management techniques including Crisis Prevention Institute (CPI) methods and CPR.


Documentation & Compliance

  • Document all client interactions and contacts according to agency and DMH standards.
  • Meet all internal and external reporting requirements including data collection for Quality Assurance evaluation and DMH reporting.
  • Attend educational seminars and professional development meetings as required.
  • Perform other duties as requested by supervisor.


Skills and Attributes:

  • Strong interpersonal engagement skills with adults, seniors, and individuals experiencing crisis or complex behavioral health needs.
  • Demonstrated ability to develop and maintain strong partnerships with law enforcement, ambulance personnel, crisis call centers, and hospital systems.
  • Highly flexible and creative - comfortable adapting your approach across a wide variety of situations and client presentations.
  • Strong clinical and crisis intervention skills with the ability to assess and respond to rapidly changing client presentations.
  • Extensive knowledge of community resources across the six-county service area and the ability to identify new resources as client needs evolve.
  • Strong verbal and written communication skills and the ability to maintain thorough, timely documentation.
  • Proficient in basic computer applications including electronic health record systems, Microsoft Word, and Excel.


Required Qualifications:

  • Master's degree in social work, nursing, psychology, mental health, human services, or a related field required.
  • Active licensure or provisional licensure in a therapeutic discipline required.
  • Minimum of two years of direct mental health experience in an inpatient, crisis intervention, or outpatient setting required.


Physical Requirements:

  • Must be able to respond to community crises in a timely manner.
  • Must be able to provide CPR and apply Crisis Prevention Institute (CPI) de-escalation techniques and other behavioral management approaches as needed.
